New CMMI Models: Implications for SDOH & Interoperability

Since December 2025, CMS has introduced four new models through the Center for Medicare & Medicaid Innovation (CMMI), targeting prevention, chronic care, accountable care, and serious illness. Together, these models signal a clear shift in how payment reform is being designed—moving toward longer-term, population-level accountability rather than short-cycle pilots or condition-specific approaches.

A key underlying assumption across these models: interoperability is already in place, data flows seamlessly, and infrastructure exists to support real-time, coordinated care.
