Building Healthier Communities Through Coordinated and Highly Collaborative Statewide Action

States across the country are leveraging health information exchange, cross-sector partnerships, and innovative payment models to address some of the nation's most pressing health equity challenges. This session brings together leaders from North Carolina, Colorado, Arizona, and New Jersey toshowcasehow coordinated data sharing, interoperable infrastructure, and collaborative quality improvement initiatives are improving maternal health, behavioral health, and social determinants of health (SDOH) outcomes. Speakers will highlight successful approaches ranging from real-time maternal risk notifications and community-based care coordination to cross-state SDOH partnerships and state-directed payment programs that incentivize measurable quality improvements.
