Single Implant Crown Masterclass: From Treatment Planning to Final Restoration

INFORMED CONSENT: By registering for this course, I hereby grant to the Faculty of Dentistry at the University of Toronto (the “University”) the right to reproduce, use, exhibit, display, broadcast, distribute and create derivative works of the images of me taken for use in connection with the activities of the University for promoting, publicizing or explaining the University or its activities.

This grant includes, without limitation, the right to publish such photographic and video images in PR/promotional materials such as University and Faculty websites, marketing presentations and  publications, advertisements, fund-raising materials and any other University and Faculty-related presentation or publication. 

These images may appear in any of the wide variety of formats and media now available to the University and that may be available in the future, including but not limited to print, broadcast and electronic/online media.
