Requests for accredited residency and fellowship training verifications must be submitted electronically via this website.  We do NOT accept physical mail, fax, email or phone requests.

For all Accredited Residency & Fellowships please click the “Submit Request" button.

To request verification of Departmental Fellowships, please email the Office of Academic Affairs at academicaffairsshare@urmc.rochester.edu.

For Internal Medicine Residency ONLY, visit the Internal Medicine verification request website.

Refund Policy: If we are unable to locate a verification for your request you will be offered a full refund. All other refunds will be subject to a $15 cancellation fee.

Please allow 10 business days from the time we receive payment for completion. However, we kindly ask that you allow up to 30 days for verifications during the busy season (May - September).
