Our Conference Story 🦋

The NCCHCA Clinical Conference began as the Janet Reaves Memorial Conference on Quality and Chronic Disease, established to honor the life and legacy of Janet Reaves, RN, MPH, a respected public health leader whose vision, innovation, and dedication helped advance chronic disease management and quality improvement efforts across North Carolina. Janet was a passionate advocate for improving health outcomes and building partnerships that strengthened care for communities throughout the state.

The conference was first held in 2009 and brought together healthcare professionals dedicated to improving quality, advancing clinical best practices, and addressing the growing impact of chronic disease. Over the years, the event expanded to encompass a broader range of clinical, operational, and population health topics relevant to community health centers and healthcare teams across North Carolina.

In 2016, the conference evolved into the NCCHCA Clinical Conference, reflecting its expanded scope and growing focus on the full spectrum of clinical care and healthcare innovation. While the name changed, the conference's mission remained the same: to provide education, foster collaboration, and support excellence in patient care.  Today, the conference continues to honor Janet Reaves' legacy through the butterfly, a symbol of transformation, growth, and continuous improvement. The butterfly serves as a lasting tribute to her influence and reminds attendees of the conference's enduring commitment to advancing quality healthcare and improving the health of the communities we serve.

Maternal Health: Explore innovative strategies, evidence-based practices, and policy solutions designed to improve maternal health outcomes and reduce disparities. Sessions will focus on prenatal care, postpartum support, maternal mental health, community-based interventions, and the critical role of care teams in ensuring healthy pregnancies and thriving families.

Care Beyond the Clinic Walls: Health happens where people live, work, learn, and play. This track highlights approaches that extend care beyond traditional clinical settings through community partnerships, social care integration, care coordination, outreach programs, and innovative models that address the social drivers of health and improve patient outcomes.

Value-Based Care: As healthcare continues its shift toward value, organizations must balance quality improvement, cost management, and patient experience. This track examines successful value-based care models, performance measurement, population health strategies, payment reform, and practical approaches that support sustainable, high-quality care delivery.

AI/ IT/ Analytics: Discover how emerging technologies are reshaping healthcare delivery, quality improvement, and decision-making. Sessions will explore artificial intelligence, predictive analytics, health information technology, interoperability, cybersecurity, data governance, and real-world applications that empower organizations to enhance patient care and operational performance.

Behavioral Health: Behavioral health is essential to overall health and well-being. This track focuses on innovative approaches to integrating behavioral health services, expanding access to care, addressing substance use disorders, supporting workforce resilience, and improving outcomes through comprehensive, person-centered care models.
