Summary
Service coordinators and other CFC personnel are invited to connect with one another and EITP staff within this forum to discuss successes and challenges that arise while partnering with families during the transition process. Participants can attend as they see fit based on their questions/needs. Before participating, we encourage you to complete a pre-survey to submit questions in advance.
Objectives
● Learn about ways to ensure smooth and effective transitions for families you serve
● Share strategies, problem-solve, and identify solutions to challenges experienced during the transition process
● Gain a sense of community and learn new ways to partner with families as they transition from/with peers and EITP facilitators
