This form is for group leaders only. All individuals must be registered through a parish group. If you are a parent or an individual attempting to register for Inspiration Midwest, please do not fill out this form. Please contact us at events@lifeteen.com.

In order to be considered the group leader, you must be attending the event with your group. If you are not attending the event, please provide the name of someone who is attending and can be the main contact during the registration process.

Please Note: The email address you provide is the email address that will be used to access your group’s digital paperwork platform. If you attempt to use the same email address for more than one registration, you will run into issues during the registration and digital paperwork process. If you need to add more tickets after submitting your initial registration, please fill out the Adding Spots Form, which can be located on our website & in your confirmation email. Do not submit a second registration with an email address you've already used as it will cause technical issues.
