AQ Duplicate Certificate Request Form

The fee for a duplicate certificate is $25.00 per copy. Any outstanding fees owing to OECTA must be cleared before duplicate certificate requests can be processed.

We will send the certificate to you only. We will not send it to a third party, nor can a third party request it on your behalf. Once you have the duplicate certificate you may make copies of the document for any agencies that require it.
