About this Live Webinar

This webinar focuses on creating a plan of care that is individualized to the patient’s personal characteristics including impairment, functional deficits and comorbid conditions, prognosis, and discharge plan. The presenter will examine CMS’s required elements for the individualized overall plan of care (IOPOC) and will explain how the physician develops this document by synthesizing information from the preadmission screen and admission assessments from all disciplines involved in the patient’s treatment.

By the end of this webinar, attendees will be able to:

  • Restate the CMS regulations for the IRF IOPOC
  • Describe documentation strategies for completing the IOPOC that will illustrate a plan of care that is unique to the patient
  • Restate the importance of the IOPOC to justify the rehab admission, especially in the absence of the PAPE
