Duplicate Certificate Request Form 2022

The fee for a duplicate certificate is $25.00 per copy. Any outstanding fee payments must be resolved in order for your request to be processed. All certificates will be sent to you, and not forwarded to other parties. We will not send it to a third party, nor can a third party request it on your behalf. Once you have the duplicate certificate you may make copies of the document for any agencies that require it.
