Our Conference Story 🦋
The NCCHCA Clinical Conference began as the Janet Reaves Memorial Conference on Quality and Chronic Disease, established to honor the life and legacy of Janet Reaves, RN, MPH, a respected public health leader whose vision, innovation, and dedication helped advance chronic disease management and quality improvement efforts across North Carolina. Janet was a passionate advocate for improving health outcomes and building partnerships that strengthened care for communities throughout the state.
The conference was first held in 2009 and brought together healthcare professionals dedicated to improving quality, advancing clinical best practices, and addressing the growing impact of chronic disease. Over the years, the event expanded to encompass a broader range of clinical, operational, and population health topics relevant to community health centers and healthcare teams across North Carolina.
In 2016, the conference evolved into the NCCHCA Clinical Conference on Quality and Chronic Disease, reflecting its expanded scope and growing focus on the full spectrum of clinical care and healthcare innovation. While the name changed, the conference's mission remained the same: to provide education, foster collaboration, and support excellence in patient care. Today, the conference continues to honor Janet Reaves' legacy through the butterfly, a symbol of transformation, growth, and continuous improvement. The butterfly serves as a lasting tribute to her influence and reminds attendees of the conference's enduring commitment to advancing quality healthcare and improving the health of the communities we serve.
Explore the Tracks Shaping the Future of Healthcare
Maternal/Women's Health: From pregnancy through the various decades of life, primary care teams play a critical role in women’s health. This track delivers practical, evidence-based guidance for addressing women’s health needs in community health centers, including maternal health, cardiovascular risk in pregnancy, cervical cancer prevention, menopause and perimenopause and care coordination. Sessions will focus on what primary care clinicians can recognize, manage, counsel and confidently incorporate into everyday practice.
Whole-Person Care: Caring for the whole person means looking beyond a single diagnosis to the many opportunities primary care teams have to improve health. This track brings practical approaches to prevention, pediatrics, oral health, pharmacy and chronic disease care into everyday practice. Sessions will equip clinicians and care teams with strategies to identify needs earlier, strengthen preventive care, expand access to essential services and deliver more comprehensive care across the lifespan.
Value-Based Care: As healthcare continues its shift toward value, organizations must balance quality improvement, cost management, and patient experience. This track examines successful value-based care models, performance measurement, population health strategies, payment reform, and practical approaches that support sustainable, high-quality care delivery.
AI-Powered Healthcare: Leverage data and technology to drive transformation. Discover how emerging technologies are reshaping healthcare delivery, quality improvement, and decision-making. Sessions will explore artificial intelligence, predictive analytics, health information technology, interoperability, cybersecurity, data governance, and real-world applications that empower organizations to enhance patient care and operational performance.
Behavioral Health: Meeting behavioralal health needs requires both strong clinical care and sustainable systems to support it. Designed for clinicians, behavioral health leaders and health center teams, this track explores practical approaches to psychiatric care in primary care, substance use treatment, expanding access in rural communities, AI in psychiatry, collaborative care and financial sustainability. Sessions will provide clinical and operational strategies that health centers can use to improve access, strengthen care delivery and build behavioral health programs that are effective and sustainable.
