2. By Tuesday, September 15th, upload your current spectacle prescription to complete registration.
3. Prescription must include:

• Doctor’s Name, Signature, and License Number
• Contact Information
• Date of the Exam
• Expiration date included and must be AFTER September 20, 2026.
• NOTE: If your prescription does not meet these requirements, NO EXCEPTIONS will be made, and we will be unable to process your eye glasses order.
• Sun lenses are excluded from this promotion.
