GW Event Management Certificate Program - certificate request form

In order to be awarded a certificate, students must submit this form and satisfy the following criteria by July 31, 2026. 

1. Successfully complete the course requirements for the certificate program - 4 core courses (4.8 CEUs) and 3.6 CEUs worth of electives.
2. Have no outstanding financial obligations to the University.

Please submit this form only if you have completed the 4 core courses and 3.6 CEUs worth of electives.
