Project failure is rarely the result of a single issue. More often, it emerges from a combination of overlooked risks, misaligned decisions, and compounding challenges across the project lifecycle. Drawing on insights from over 500 real-world projects analysed by the International Project Leadership Academy, this session explores the recurring patterns that lead to underperformance and failure in complex engineering projects. Through practical examples and industry war stories, the presenter will unpack common failure triggers across planning, contracting, stakeholder management, and delivery. Attendees will gain a structured way to recognise early warning signs and apply lessons learned proactively. This session is designed to support engineers in strengthening project outcomes by embedding reflective practice and continuous improvement into both project start-up and close-out phases.
