HCPs: If requested, a hotel reservation has been made on your behalf. The night of Sunday, June 14 will be paid directly by Shockwave and an ID and personal credit card will need to be provided at check-in for incidentals. Any additional hotel nights requested are the responsibility of the individual and cannot be paid by Shockwave without prior approval.
Shockwave Employees: Please make sure you have booked a hotel room (if needed) at the Hilton Garden Inn and submit expenses through Concur.

IMPORTANT REMINDERS
Sunday, June 14: Please meet in the hotel lobby at 5:30pm for registration and departure to the dinner at 6:00pm.
Monday, June 15: Please meet in the hotel lobby at 7:00am for departure to the hospital. Please check out of your hotel room and bring your luggage to the course on Monday.
Local attendees (or anyone with a vehicle) - Please drive separately to the hospital on Monday. Do not board the shuttle.

GROUND TRANSPORTATION
HCP's: Uber vouchers will be emailed to you prior to the course for use to/from the airport.
We will rideshare from the hotel to dinner on Sunday evening and then again on Monday morning from the hotel to the course. Please check out of your hotel room and bring your luggage to the course on Monday.
REGISTRATION POLICY
Attendance for this program and all associated agenda functions are limited to registered Health Care Professionals and Shockwave Employees only. Unregistered guests are not permitted to attend.
EXPENSE REIMBURSEMENT
Some qualifying expenses may be reimbursed by Shockwave upon conclusion of the course. Please carefully review page 2 of the form below for details and instructions on how to submit.
