Assumption of Risk and Release

I recognize that there are certain inherent risks associated with any activities that I may participate in related to this event (the “Program”), and I assume full responsibility for personal injury, illness, death, medical treatment to myself, and hereby release, discharge, and agree to indemnify and hold Creative Group, Inc. (“Creative Group”) and Trove Information Technologies, Inc. dba Pave (“Program Sponsor”), or their respective parent, subsidiary or affiliated companies and their respective officers, directors, employees, independent contractors and agents (all collectively, the “Releasees”) harmless for any injury, loss or damage arising out of my use of or presence upon the facilities or from any activities arranged by the Releasees at my direction and request, whether caused by the negligence of myself, the Releasees or other third parties providing services in connection with the Program.

Photo, Video, and Likeness Consent

By attending this event, I acknowledge that Creative Group and Trove Information Technologies, Inc. dba Pave (“Pave”) may capture photographs, video recordings, and audio recordings of event attendees (collectively, “Event Media”).

I hereby grant Creative Group and Pave an irrevocable, royalty-free, worldwide license to use, reproduce, publish, display, distribute, and create derivative works from any Event Media in which I appear, in connection with the marketing and promotion of Pave’s products, services, and events, including but not limited to use on Pave’s website, social media channels, email communications, print materials, presentations, trade publications, and paid advertising.

I understand that I will not receive compensation for such use, that I waive any right to review or approve the final materials, and that my name and identifying information may or may not accompany the Event Media.

If I do not wish to be photographed or filmed, I understand that I should notify event staff at registration or at the event entrance and that reasonable efforts will be made to accommodate that request.
