I authorize TeenPact to use any statements, photos, or videos of the attendee for fundraising or promotional material in support of the TeenPact ministry. I acknowledge that TeenPact is not responsible for the loss or damage to property brought by the attendee to a TeenPact event, including but not limited to cell phones, laptops, tablets, and other valuable items. I recognize that I am responsible for the supervision and proper use by the attendee of his or her electronic or photographic devices and release TeenPact from any liability which may result from the attendee's misuse of said items.

If the parent, guardian, or emergency contact for the attendee is unable to be reached prior to the attendee needing emergency treatment following an incident, I hereby give permission to a TeenPact Director and/or State Coordinator to approve x-rays, routine tests, hospitalization, anesthesia, and/or surgical procedures or emergency treatment for the attendee. Further, I hereby authorize the release of any and all medical information concerning the attendee to appropriate medical personnel and/or any appropriate healthcare provider or insurance company. In consideration for being permitted to participate in the TeenPact program, I agree to release TeenPact Leadership Schools, including its directors, volunteers, agents, or facilities, from any liability for any claims or damages related to injury, death, or property damage resulting from the transportation or housing of the attendee during a TeenPact event.
