I understand that this event is voluntary and that I may decline to participate without adverse consequences in my employment.  I understand that I will not be asked to perform work functions at the event.  I agree to assume all risks incidental to or arising from my taking part in the event.  The risks may include, but shall not be limited to, physical injury, damages, or other harm.  I further attest that I have overall good health and that I do not have any physical or psychological condition that would create an unreasonable risk of harm to myself or others in participating in this event. On my own behalf, I release and forever discharge St. Jude Children's Research Hospital, Inc., its Board of Governors, officers, employees, agents and assigns from any and all liability of any nature whatsoever arising out of or related to my taking part in this event. Please note the following considerations: Participation in the event is voluntary. Participation in an after-traditional work hours event is voluntary. Declining participation in this event will not affect employment. Participation in the event will not include work functions. Voluntary participation in this event removes all liability from St. Jude Children’s Research Hospital. Participantattests to being in good health condition to participate in the event. All body contact is voluntary, and participant may refuse or request limited body contact during event.
