Please submit this Attendance Reporting Form by the due date, with accurate information. The event planning team will ensure accurate records (based on what you provide) are submitted to the correct entity for your credits.

Note: You have until November 12, 2026, to submit your reporting form. If you fail to submit by the due date or provide inaccurate information, it is the attendees’ responsibility to self-report and make corrections.

No late submittals will be accepted.

Individual certificates will be emailed to attendees after the seminar once all the data has been collected and certificates are generated.  This process can be lengthy since each attendee receives a personal email with their certificate.  If you are applying for multiple CE credit those certificates will come in waves.
