Request for Duplicate Certificate (FY26)
Please click the 'Request Duplicate Certificate' button to request a copy of your certificate(s) of attendance/participation in events sponsored by the Early Intervention Training Program at the University of Illinois.
When placing your order, you will need the following information:
- Name of Event
- Date of Event
- Location/City of Event
- Name of Participant as it appeared on the original certificate (must be the same as registrant)
- Email address to send the duplicate certificate(s)
Processing Fees
This request is for certificates originally issued by the Early Intervention Training Program at the University of Illinois that have become lost, destroyed or otherwise.
Original certificates with an error, (misspelled name, incorrect date, credit information, etc.), will be replaced at no charge to the participants. All other requests for duplicate certificates are at a cost of $10 per certificate.
PLEASE NOTE: EITP can only issue duplicate certificates for events sponsored by EITP
