Overview
Missouri hospitals are making significant strides in population health — but turning those insights into action is what will define success in the next era of health care.
This virtual session will review the 2026 Population Health Assessment Survey Report, MHA's fourth triennial assessment and the most comprehensive snapshot yet of population health maturity across the state. Drawing on responses from 120 Missouri hospitals, the report examines value-based care readiness, community engagement, workforce challenges and efforts to address health disparities.
This session coincides with the release of the statewide comprehensive report and hospital-specific reports, giving participants a chance to benchmark their organization against statewide trends and identify targeted opportunities for improvement.
MHA staff will walk through key findings, highlight what high-performing organizations are doing differently, and share practical strategies for strengthening leadership, workforce capacity, data infrastructure, care coordination, and community partnerships — followed by live Q&A.
Objectives
Upon completion of this program, participants will be able to:
- Analyze findings from the 2026 Population Health Assessment Comprehensive Report and their hospital-specific report to understand statewide population health trends, benchmark organizational performance, and identify strengths and opportunities across the nine core domains of population health maturity
- Examine the leadership, governance, financial, operational, and community engagement characteristics associated with high-performing hospitals and health systems and compare these findings to their own organization's assessment results
- Assess organizational readiness for value-based care using statewide benchmarks and hospital-specific findings to identify strategies that strengthen financial sustainability, care coordination, risk-based contracting and participation in alternative payment models
- Recognize opportunities to leverage data, technology, and community partnerships by evaluating assessment findings related to interoperability, population health analytics, social needs integration, closed loop referrals, and cross-sector collaboration within their organization and across Missouri hospitals
- Explore practical approaches for addressing workforce challenges and health disparities by using both statewide and hospital-specific data to identify gaps, prioritize interventions, strengthen workforce capacity, reduce burnout and better address nonclinical factors that affect an individual's health
- Develop an actionable road map based on insights from the comprehensive statewide report and their hospital-specific assessment report to establish priorities, set measurable goals, improve outcomes, reduce health disparities and advance organizational population health maturity
