I hereby recognize and acknowledge that there are inherent hazards and risks connected with the Outdoor Adventure. On behalf of myself or the child being registered, I (1) Knowingly and voluntarily assume full responsibility for such risks and hazards and (2) Agree that I am bound by all terms and conditions of the Outdoor Adventure rules and procedures. I am aware of the content of the Outdoor Adventure and hereby represent that myself or my son or daughter is physically, mentally and emotionally fit and capable of safely participating in the Outdoor Adventure. I agree as a condition of participation in this program, to hold its manager, directors, staff and volunteers harmless from and against any and all loss, judgments, damage and expense incurred by reason of any claim or liability based upon (1) Personal injury (including death) or property damage to any person or building arising out of the negligent or intentional action of the youth attendee, or (2) Personal injury (including death) or property damage to the youth attendee, except to the extent that such injury or damage is directly caused by the acts or omissions of the Outdoor Adventure, Ensign Peak Academy, its manager, directors, staff and volunteers.
