NOTICE TO ALL ATTENDEES: Please note that travel, lodging and incidentals are not included in your registration and are your responsibility. Hyland is offering (1) food and refreshments at the Hyland Healthcare user forum at an estimated fair market value of $40, and (2) food and refreshments at the networking event at an estimated fair market value of $35.00 (together, the “event”), at no cost to attendees. By attending, you certify that your attendance, including your participation in any food, refreshments, giveaways, ancillary events and other items made available at no charge: (i) is approved pursuant to your employer’s policies — whether paid or complimentary — and (ii) does not create any violation of your employer’s gifts and ethics rules or any contract, statute, law or industry regulations. You also may pay the fair-market value or those portions requiring payment in accordance with your gifts and ethics rules or any contract, statute, law or industry regulations applicable to you (please reach out to Lorna.Green@hyland.com for details on making payment).

NOTICE TO GOVERNMENT/PUBLIC SECTOR EMPLOYEES, OFFICIALS AND REPRESENTATIVES: If you are a government sector employee, official or other representative, in order to attend you must do one of the following: (1) Have the authorization form completed, executed and returned to Hyland in accordance with its instructions; (2) Pay the estimated fair-market value or those portions requiring payment via check to Hyland Software, Inc. prior to attending (please reach out to Lorna.Green@hyland.com for details on making payment); or (3) Identify yourself as a government or public sector employee/official/representative and not partake in any food, refreshments, entertainment or giveaways.
