Workshop Description
The Behavioral Health system of care has shifted the focus from provider centered care planning to a collaborative shared decision-making process involving the individual in recognition of their unique strengths, goals, preferences, culture, and definition of recovery. This shift requires providers and systems to adjust how care plans are created and services are delivered. This course provides a basic understanding and framework for person centered care planning across disciplines and roles.
Learning Objectives:
- Define person-centered care planning and describe the evolution from provider-directed treatment approaches to recovery-oriented, trauma-informed systems of care for individuals with mental health and substance use conditions.
- Apply the principles of person-centered care planning across disciplines and service settings, including prevention, treatment, harm reduction, and recovery support, through case-based examples.
- Integrate person-centered care planning practices that promote shared decision-making, integrated care, and whole-person care within behavioral health systems of care.
Audience: Recovery coaches, social workers, allied mental health providers, alcohol and drug counselors. Priority population Substance Use Disorder System of Care
