Please be aware that although you are registering online, your registration will not be considered complete until we have received the following items from you by mail or email.

  1. School Registration Online Form: Completion of this online registration form.
  2. Medical/Media Release Forms (Word Document): Complete, sign, and submit by email or mail.
  3. Principal Acknowledgment Form (Word Document): Complete, sign, and submit by email or mail.
  4. List of All Student & Chaperone Attendees (Excel): This list must be submitted the same time as the Medical/Media Release Forms.
  5. Full Payment: Submit full payment via credit card (add a 5% service fee), a single check payable to University of Illinois, or a purchase order (PO).
