By registering and/or attending an Event and emails related to an Event, Cardinal Health or Cvent, on our behalf, is or may be collecting the following Person Information from you: Full Name, Company, Title, Email Address, Mailing Address, and Phone Number. We may also ask our Health Care Provider registrants to share information such as NPI number, license status, and state of licensure. The collection of your Personal Information will be used in connection with an Event to: Communicate with you regarding the Event for which you registered or future Events; for our internal reporting and/or planning purposes; and as reasonable necessary for the management and organization of the Event for which you registered, including the reservation or scheduling of lodging, transportation, or other logistical requirements you may require.
