Person-directed care plans are often reduced to using the pronoun “I” and adding preferences, priorities and goals. Yet they have the potential to support relationships and describe an elder’s legacy while meeting regulations. In fact, care plans can be so meaningful that content is read at a birthday party or included in a memorial service.  

Using The Eden Alternative’s GROWTH Model (a quality assessment performance improvement process), communities will evaluate their care plans process based on the Artifacts of Culture Change Definitions and Measurement #52.
