This leadership certificate program is currently open to active duty, reserve, and national guard nurse service members. Note: Civilian nurses working in the DoD are not eligible. Participants who successfully complete the course work will earn a graduate certificate in Leadership & Critical Thought from Cornell University.

The application requires a short essay (250 words) consisting of the goals for participation, and the identification of one PhD or DNP mentor and one mentor with a graduate degree (Master's or higher). One of the mentors must be a nurse.    Mentors may be civilian, federal, or military.  Participants must meet quarterly with their mentors regarding their project progress.  Upon completion of the program all military nurse participants must provide the course attendance record, progress report (storyboard) of their identified project, and complete the participation survey.

I agree if accepted into this program that I will complete the program in a satisfactory manner to earn the TSNRP  Fellowship Leadership & Critical Thought Certificate. Upon completion I agree to complete a project at my assigned duty station, and submit a storyboard to TSNRP of the project. I understand I will have up to 18 months to complete this online program which consists of the following virtual 4 core courses at eCornell (1. Problem-Solving Using Evidence and Critical Thinking, 2. Strategic Decision Making, 3. Making Strategic Change Happen and Improving Quality and Performance of Healthcare Services) plus 1 elective selected among 19 elective courses. Each course is provided online over 10 hours during a two week period). In the event, I elect to withdraw, I agree to notify, in writing, the TSNRP Executive Director. I understand that if I do not affirm the attestation, I am not eligible for this program.  Therefore, I affirm the attestation.
