The session will also review CMS's recent changes to the documentation requirements serving as the basis of payment for IDT conferences.

  • Define CMS’s documentation requirements for the individualized overall plan of care (IOPOC) and weekly interdisciplinary team conference notes, as outlined in the Medicare Benefit Policy Manual.
  • Describe effective strategies for creating a unique IOPOC for each patient and ensuring that it accurately reflects the patient’s specific conditions and needs.
  • Recognize whether their facility’s weekly team conference notes meet CMS’s requirements, thereby ensuring compliance and improving documentation quality.
