- Define CMS’s documentation requirements for the individualized overall plan of care (IOPOC) and weekly interdisciplinary team conference notes, as outlined in the Medicare Benefit Policy Manual.
- Describe effective strategies for creating a unique IOPOC for each patient and ensuring that it accurately reflects the patient’s specific conditions and needs.
- Recognize whether their facility’s weekly team conference notes meet CMS’s requirements, thereby ensuring compliance and improving documentation quality.
