AAFP Translation to Practice | T2P

The AAFP's Translation to Practice® (T2P®) is a post-activity, implementation-based learning exercise designed to reinforce learning and support meaningful practice change.
AAFP members can earn two additional Prescribed credits per qualifying session by applying what they learned in CME—such as new guidelines, clinical strategies, or techniques—directly into their practice.
Participation is optional but provides a practical way to translate conference learning into everyday patient care while earning additional CME credit.
There are three steps required for each T2P opportunity.
- Step 1: Commitment to Change | Part One Survey
Complete a brief “Commitment to Change” statement indicating what practice change you plan to implement based on the session. This is done by completing a Part One T2P survey (provided by the NCAFP) for each opportunity you choose to participate in. - Step 2: Implement the Change
Over the next 30 to 45 days, implement the selected changes in your clinical practice. - Step 3: Report the Change | Part Two Survey
After 30 to 45 days, the NCAFP will email you a link to a Part Two T2P survey. In this follow-up survey, you will report whether the change was implemented, describe any impact on patient care, and identify any barriers or challenges you encountered. You must complete the Part Two Survey to earn the extra CME credit.
The NCAFP will report T2P credits earned by all participants to the AAFP. T2P credits cannot be self-reported by members. Non-members may not participate.
